Sharing a prayer request is your choice. You can use the resource library without submitting anything.

What you share and who receives it

The form sends your prayer request, any name and email you provide, your follow-up preference, and your consent to the YP prayer coordinator at cyprianmoses4@gmail.com. Name is optional. Email is required if you ask for a reply. The request is intended for prayer and any follow-up you request.

Requests are not published on this website. Please share only what you are comfortable sending by email, and avoid identifying details about other people.

Delivery and storage

FormSubmit processes and emails submissions, uses spam verification, and says it retains a copy of submissions for 30 days. Copies in the coordinator’s email inbox have separate retention; the 30-day period does not automatically delete those emails. See FormSubmit’s privacy policy for its practices.

The website does not save prayer drafts in browser storage or keep its own prayer-request database. Hosting, security, email, and verification providers may process technical information such as IP addresses and use cookies to operate their services. YouTube thumbnails and campus images load from external providers; their own privacy policies apply.

Questions and deletion requests

Contact the prayer coordinator to ask about how your request is used or to request deletion of an emailed request. Please provide enough information to identify your request without repeating sensitive details. Requests relating to FormSubmit’s records may also require contacting FormSubmit.

A reply may take time. For personal support, visit Menlo Church’s care page. This prayer form is not an emergency service. For immediate emotional or crisis support in the U.S., call or text 988. In immediate danger, call 911.
